YOUR VISIT

 

Your initial visit with Proliance Puyallup Surgeons begins with a conversation about you. We like to get to know you, learn about your life and how you live it. During this conversation, your surgeon will talk with you about your medical health history, ask questions about your symptoms and events related to your illness prior to a physical examination. 

A physical exam is part of the initial visit process. Your surgeon will have reviewed chart notes, prior to your visit, from a referring physician. This will help your surgeon perform a meticulous exam and prepare a diagnostic report if indicated.  Your surgeon may request that you complete laboratory and radiological studies to empower the most precise diagnosis and provide you with the least invasive treatment options available.

Please bring the following information to your first visit:

 
  • Referring/Primary Care
  • Physician Information
  • Valid Photo ID
  • Primary & Secondary
  • Insurance information and card
  • Emergency Information
  • Work Information
  • A list of your current medications
  • All x-rays film
  • Information about what treatments and medications have been tried in the past
  • Medical records from prior visits that you think may be helpful

If surgery is the best course of action for your needs, we will schedule it at your earliest convenience. Many conditions can be treated in our Outpatient Surgery Center, but every patient is different, and depending on your unique situation you may require more extensive surgical procedures or treatment at MultiCare Good Samaritan Hospital. If you require surgery, the type of surgery, potential complications, recovery, and outcomes will be discussed with you at this time. Your surgeon will give you instructions regarding preparation for your procedure.  

After surgery, we'll guide your recovery, provide follow-up care and work closely with your referring physician so you can get back to your life, faster.

 

Cancellation


Cancellation of an appointment is sometimes unavoidable due to medical problems or significant conflicts which cannot be avoided. Please provide us with at least 24 hours advance notice if you need to cancel or reschedule an appointment. We may charge a $50.00 fee for missed appointments.

Please provide us with at least 48 hours advance notice if you need to cancel or reschedule an appointment that requires an interpreter. Appointments requiring an interpreter canceled or rescheduled within 48 hours may be charged for the interpreter.

All cancellations need to be made over the phone.
